Re-Entry CES Case Manager

A coordinated entry case manager works with a network of  partners to streamline the process for people experiencing homelessness to access housing and supportive services. The case manager's role involves using standardized assessments to match a client's needs with available community resources, eliminating the need for them to navigate the complex system alone. Specifically to re-entry CES Case Manager will provide case management focused on career/employment goals to justice-individuals. Assistance with the implementation and management of a Coordinated Entry System specific a reentry project. Working closely with the agency and its six project partners to provide variety of housing, wraparound care, and employment services.

The Certified STRTP/GH Administrator is responsible for ensuring that residential youth are treated fairly and respectfully while residing in a safe, home-like environment where they receive treatment. Under the supervision of the Director of Youth Shelters, provides day-to-day management and care to residents in a state-licensed Short-Term Residential Treatment Program. Responsible for hiring, training, scheduling, and supervising Residential Advocates who provide daily care for residents. Maintain program and facility regulatory compliance. Works closely with Case Managers to ensure all aspects of residents’ mental health treatment plans are implemented and followed. Manages daily house operations and, when necessary, maintains positive relationships with the neighborhood.
