POSITION: Case Manager 2 (Reentry Services)

REPORTS TO: Program Manager

HOURS: This is a full-time position, includes some weekend and evening shifts.

BASIC DUTIES: Work independently and as part of a team, providing holistic and intensive case management services to justice-involved populations. Including intake assessment, benefit assessment, goal setting, weekly case management development, progress monitoring, advocacy, and referrals.

AREAS OF RESPONSIBILITY

PROGRAMMING

  • Provide intensive case management for soft-employment skills and wraparound care.
  • Assist clients with obtaining, maintaining, and thriving in a quality career.
  • Assist with job searches, mock-interviews, and linkage to on-the-job training.
  • Implement and track participant support stipends funds for client needs.
  • Conducting client intakes and assessments to gather information.
  • Meeting with clients weekly, creating and maintaining individual client goals.
  • Connect clients with needed and available community resources.
  • Ability to motivate others towards achieving goals.
  • Provide all required documentation in a timely manner, which may include client follow-up, outcome evaluation, and evaluation.
  • Follow-up with client and agencies to document use/success of referral.
  • Coordinating wraparound services including.
  • Addresses client issues and concerns, works as a liaison with shelter manager.
  • Ensure that all documents submitted on behalf of a client are valid.
  • Input client data into Cap60.
  • Provide all required documentation in a timely manner.
  • Other duties as assigned.

MINIMUM REQUIREMENTS

  • Knowledge of community resources and counseling/social work practices with high risk population.
  • Excellent documentation skills.
  • Excellent written and verbal communication skills, ability to establish rapport.
  • Pass a background check.

The Certified STRTP/GH Administrator is responsible for ensuring that residential youth are treated fairly and respectfully while residing in a safe, home-like environment where they receive treatment. Under the supervision of the Director of Youth Shelters, provides day-to-day management and care to residents in a state-licensed Short-Term Residential Treatment Program. Responsible for hiring, training, scheduling, and supervising Residential Advocates who provide daily care for residents. Maintain program and facility regulatory compliance. Works closely with Case Managers to ensure all aspects of residents’ mental health treatment plans are implemented and followed. Manages daily house operations and, when necessary, maintains positive relationships with the neighborhood.
