Position Overview

The Case Manager for Transitional Aged Youth (TAY) provides intensive case management services to young adults (ages 18-24) residing in supportive housing programs. The primary goal is to help participants obtain and maintain stable housing while building the skills and confidence needed for independent living. The Case Manager provides individualized support, resource coordination, and advocacy in alignment with the Housing First model.

Essential Responsibilities

  • Conduct intakes, assessments, and individualized Housing Plans for eligible TAY participants in supportive housing.

  • Assist youth in obtaining and maintaining stable housing, including landlord engagement, move-in coordination, and ongoing support to sustain housing.

  • Provide education and coaching on life skills, including budgeting, household management, employment readiness, and independent living skills.

  • Maintain accurate and timely documentation in HMIS and agency databases.

  • Connect participants with employment, education, healthcare, and community resources.

  • Collaborate with agency staff and community partners to ensure comprehensive, coordinated support.

  • Participate in team meetings, supervision, and trainings as required.

Qualifications

  • High school diploma or GED required; bachelor's degree in Social Work, Human Services, or related field preferred.

  • One (1) year of experience in case management or social services preferred.

  • Experience working with Transitional Aged Youth or individuals experiencing homelessness strongly desired.

  • Strong communication, organizational, and documentation skills.

  • Proficiency in Microsoft Office, Outlook, Zoom/Teams, and HMIS or similar databases.

  • Valid California driver's license, reliable transportation, and auto insurance required.

  • Bilingual (English/Spanish) preferred.

Work Environment

This position requires travel between program sites and community locations. The work involves both office and field settings and may include occasional evenings or weekends. Reasonable accommodations will be made for individuals with disabilities.
