Re-Entry CES Case Manager

AREAS OF RESPONSIBILITY

PROGRAMMING

  • Assist with the development and implementation of an ongoing system for coordinating swift and seamless referrals between all project partners.
  • Oversee a Coordinated Entry System which includes the agency and project partners.
  • Provide intensive case management for soft-employment skills and wraparound care.
  • Assist clients with obtaining, maintaining, and thriving in a quality career.
  • Assist with job searches, mock-interviews, and linkage to on-the-job training.
  • Implement and track participant support stipends funds for client needs.
  • Conducting client intakes and assessments to gather information.
  • Meeting with clients weekly, creating and maintaining individual client goals.
  • Connect clients with needed and available community resources.
  • Ability to motivate others towards achieving goals.
  • Provide all required documentation in a timely manner, which may include client follow-up, outcome evaluation, and evaluation.
  • Follow-up with client and agencies to document use/success of referral
  • Plan and manage CES meetings with personnel and project partners.
  • Ensure data is captured and input into an online database system (i.e. apricot).
  • Assist with screenings, assessments, and verifications for project services.
  • Ensure best practices for information gathering, client placements, and confidentiality.
  • Provide CES data and documentation for reports in a timely manner.
  • Other duties as assigned.

MINIMUM REQUIREMENTS

  • Knowledge of community resources.
  • Excellent documentation skills.
