We are seeking a dedicated and compassionate Case Manager to join our Project-Based Housing team. In this role, you will provide outreach, case management, housing navigation, and supportive services to help participants move from homelessness to stable housing-and stay housed.
Conduct in-person outreach and complete intake processes for referred participants
Develop housing and income plans tailored to each participant's goals and barriers
Identify housing opportunities, including shared housing
Build strong relationships with landlords and property managers
Maintain timely and accurate case notes across data platforms
Ensure lease-up compliance and maintain program documentation
Collaborate with clinical and community partners to support housing retention
Attend regular team meetings, trainings, and interagency coordination sessions
Provide occasional transportation to participants for appointments
Experience working with individuals experiencing homelessness, including those with mental health challenges, substance use issues, and trauma histories
Knowledge of trauma-informed care, crisis intervention, and harm reduction
Strong communication, organizational, and problem-solving skills
Proficiency in Microsoft Office (including Excel), HMIS/Clarity, and case documentation
Ability to work independently and within a multidisciplinary team
Valid California driver's license, clean driving record, and reliable transportation
Willingness to work flexible hours, including evenings and weekends if needed
Must pass a background check
Experience navigating housing markets and working directly with landlords
Understanding of housing subsidies and lease-up processes
