Position Overview

Family Assistance Program is seeking a highly motivated and detail-oriented Case Manager to join our Next Step Reentry Services for PROWD team. The Case Manager will provide intensive, client-centered case management to justice-involved individuals with the goal of supporting successful reintegration, long-term stability, and sustainable employment. This position requires strong interpersonal skills, sound judgment, and a demonstrated ability to work both independently and collaboratively.

Key Responsibilities

  • Conduct comprehensive client intake assessments and develop individualized service plans.

  • Facilitate employment readiness by assisting clients with job searches, interview preparation, soft-skills development, and referrals to vocational training.

  • Administer and monitor participant stipends in accordance with program guidelines.

  • Provide consistent case management, including weekly client meetings, progress monitoring, and goal adjustments.

  • Coordinate and track referrals, ensuring effective linkage to community resources and wraparound services.

  • Maintain accurate and timely case documentation, outcome evaluations, and data entry in Apricot.

  • Serve as an advocate for clients, addressing barriers to reentry and fostering motivation to achieve personal and professional goals.

  • Collaborate with internal staff, partner agencies, and community organizations to enhance client support and program outcomes.

  • Perform additional duties as assigned to advance program objectives.

Minimum Qualifications

  • Knowledge of community resources and best practices in social services, counseling, or case management with high-risk populations.

  • Strong written and verbal communication skills with the ability to establish rapport and maintain professional boundaries.

  • Excellent organizational, documentation, and time management skills.

  • Ability to work with diverse populations and adapt to dynamic client needs.

  • Must successfully pass a background check.
